Enrollment,

In order for your children to become enrolled at Karsten Kiddie Kare there are a few steps that must be taken. First we require a face-to-face interview with the parents/guardians and child before a decision is made to whether or not care can be provided. The state requires a number of forms to be fillled out before the first day of attendance. One of the forms that is needed is an Illinois Department of Health Medical Exam, which must be signed by a doctor and current within six months of your child's admission. This form is required every two years until school age and then upon entry into kidergarten and fifth grade. Other forms required are:

1. Application/Record of child information
2. Consents to daycare providers
3. Certificate of Child Health Examination
4. Copy of child's birth certificate
5. Parent/Gaurdian contract
6. Day Care Resources, Inc. - Child registration form
7. Signed Policy Book
8. Signed DCFS summary standards booklet
9. Medication Log forms
10. Transportation/Walking To and From School Consent (School Aged Children Only)

Most of these forms can be found on the right, however some need to be provided by you (birth certificate) or will be provided during our interview.
